Schedule an Appointment & Complete an SOA

After you schedule a time:

You’ll be asked to complete a short Medicare-required Scope of Appointment form (SOA).
 

This form lets me legally review your plan options.

What Is a Scope of Appointment (SOA)?


The Scope of Appointment (SOA) form is required by Medicare to document your permission for an agent to discuss specific types of Medicare plan options with you.

This form does not obligate you to enroll in any plan — it simply protects you by outlining what will and won’t be discussed during our meeting.
 

Per CMS guidelines, the SOA must be completed before we talk about plan benefits, pricing, or coverage details.

✅ You can fill it out online after scheduling your appointment.
✅ No personal doctors, prescriptions, or financial info is required to complete it.
✅ You can cancel or reschedule your appointment at any time.
